Uninsured Services

Filling out forms are not covered by the Medical Services Program.

Dr. Konkin charges fees reflective of current Doctors of BC rates for uninsured services.

• A00061 Written certificate, including time loss benefit form (extra to examination).                   - Docs of BC recommendation - $52.90   (Dr. Konkin's fee $40.00)

• Written note for school or brief note with office letterhead - no charge
